Each and every one of us is unique in our own way. Similarly, your child too may have a unique set of challenges, likes or dislikes. Please download the template to record your child’s strengths and challenges. So they can get the help they seek from the people interacting with him/her at home or outdoors.

Use this printable form to record your child’s strengths and core issues he/she is dealing with on a regular basis.

What are the  advantages of having a template to record a child’s strengths and challenges, you may ask?

1. If your child is non-verbal then having a unique list of his/her habits and strengths can help in effective communication of his/her needs to the people he/she interacts with.

2. This document also has the contact details of the child. It will be useful incase the child wanders or gets lost in a crowd.

3. If the child is asked to name his/her school or other personal details, and is unable to do so, in which case this document would come in handy.

4. Specifically highlighting your child’s core strengths/difficulties will help his teachers and friends understand him/her better. Also will prompt them to respond accordingly.

5. Last but not the least, a deeper knowledge of how a child may react or respond to any given situation will drive sensitization. And a sense of inclusion among people interacting with the child.

Please ensure this document remains in your child’s backpack at all times.

Additionally, please do explain the utility of the document to your child. Thich will also prompt him/her take responsibility for it and safe guard the document at all times. Along with the knowledge of instances to hand it out to people.

You may also download the Antecedent Behavior Consequence (ABC) chart to document your child’s behavioural concerns.
